Summary
A 10-week parallel-arm RCT in overweight/obese adults consuming 25% of energy requirements from either fructose- or glucose-sweetened beverages. Both groups gained similar weight, but increased visceral adiposity, elevated fasting glucose and insulin, and decreased insulin sensitivity occurred only in the fructose group — a landmark study showing that the specific sugar (fructose vs glucose) shapes metabolic outcomes.

A meta-analysis pooling 4 intervention studies (9 arms, 767 participants total) comparing HFCS and sucrose. No significant differences were found for body weight, BMI, waist circumference, fat mass, blood pressure, fasting glucose, or lipid profile (triglycerides, LDL, HDL, total cholesterol). CRP (an inflammation marker) was slightly higher in the HFCS group.
Effect of Fructose on Non-Alcoholic Fatty Liver Disease Markers: Isocaloric Exchange vs. Hypercaloric Excess (Meta-Analysis of Controlled Feeding Trials)

A meta-analysis of 8 reports (13 controlled feeding trials, 260 healthy participants). Isocaloric fructose-exchange trials (7 trials) showed no change in liver fat markers, while hypercaloric trials providing excess fructose (6 trials, +104-220 g/day, +21-35% energy) significantly increased intrahepatocellular lipids and ALT. The authors concluded the effect may be more attributable to excess energy than to fructose per se.
