"High-fructose corn syrup is worse for your liver than regular sugar" is a claim you'll see often on social media and health sites. Research linking fructose metabolism to fatty liver certainly exists, but much of it doesn't directly compare high-fructose corn syrup against sucrose (table sugar). Here's what the evidence actually shows, accounting for the differences between studies.

Does HFCS specifically drive fatty liver more than sucrose (regular sugar)?

High-fructose corn syrup has a higher fructose content than sucrose, and fructose is converted directly into fat in the liver, so it causes fatty liver and impaired liver function more readily than regular sugar.
What research says
- Sucrose is a disaccharide with glucose and fructose bound 1:1, and after digestion it's absorbed in essentially the same form as HFCS (roughly 42-55% fructose).
- A meta-analysis pooling 767 participants found no significant difference between HFCS and sucrose in metabolic markers such as weight, blood glucose, or lipids.
- Separately, a meta-analysis of 13 trials (260 participants) examining fructose's effect on fatty liver found no change in liver markers when fructose isocalorically replaced other carbohydrates — liver fat and ALT rose only in trials where fructose was consumed in excess of total energy needs.
At matched intake, there's little evidence that HFCS is specially harder on the liver than regular sugar. What drives liver impact is total caloric intake exceeding needs, more than the specific type of sugar.
Does habitually drinking a lot of sugary beverages burden the liver, regardless of sugar type?

If HFCS and sucrose don't differ, then drinking as much juice or sports drink as you want shouldn't affect the liver at all.
What research says
- In an RCT where overweight/obese adults consumed 25% of energy needs from fructose- or glucose-sweetened beverages for 10 weeks, both groups gained similar weight, but increased visceral fat and reduced insulin sensitivity occurred only in the fructose group.
- The meta-analysis above also found that excess fructose beyond total energy needs (+104-220 g/day, +21-35% energy) significantly raised liver fat and ALT.
- Even if the sugar-type difference is small, habitually overdrinking sugary beverages to the point of excess total calories can still burden the liver.
Whether it's HFCS or sucrose matters less than whether total calories exceed your needs. Habitually overdrinking juice or sports drinks to the point of caloric excess raises risk regardless of the specific sugar type.

Fructose-glucose syrup is a liquid sugar made by enzymatically converting part of the glucose from corn (or other) starch into fructose. In Japan, JAS standards classify it into three types by fructose content: 'glucose-fructose syrup' (under 50% fructose), 'fructose-glucose syrup' (50-90%), and 'high-fructose syrup' (90%+). It stays sweet and dissolves easily even when cold, which is why it's widely used in soft drinks and sports drinks served at room temperature or chilled.

Does the Fructose-Glucose Syrup in Sports Drinks Help During Exercise?
Glucose and fructose are absorbed via separate intestinal transporters, so combining them lets the body use more carbohydrate faster than a single sugar source alone. A single carbohydrate's utilization rate (oxidation) caps at roughly 60 g/hour, while a glucose-fructose mix can push this to around 90-105 g/hour. Most of this evidence, however, comes from prolonged endurance exercise lasting over 2 hours, and doesn't necessarily transfer directly to a shorter resistance-training session.
