Does Creatine Affect the Brain? What the Research Says About Cognitive Function

I've heard creatine affects the brain — does it actually make you sharper?
Creatine isn't just a muscle fuel — it's also part of the brain's energy system. Research reports improvements on some memory and processing-speed tasks under sleep deprivation, and in vegetarians, whose dietary creatine intake is low. In well-rested, healthy young adults, the effect is small and inconsistent. It isn't a "smart drug."
Why creatine would matter for the brain in the first place
Creatine isn't only a muscle fuel — it's also used in the brain. Despite making up only about 2% of body weight, the brain is thought to consume roughly 20% of the body's total energy expenditure, making it a highly energy-demanding organ. As creatine phosphate, it acts as a buffer that rapidly regenerates ATP (the cell's energy currency) — that's the mechanistic starting point for research into whether topping up creatine helps when the brain's energy demands spike.
Under sleep deprivation, even a single dose improved some task scores
In a double-blind, placebo-controlled crossover trial with 21 hours of total sleep deprivation, a single dose of creatine significantly improved processing speed on language, logic, and numeric tasks, plus word-memory performance, versus placebo. But the dose used was 0.35 g/kg body weight (roughly 26 g for a 75 kg person) — far more than the typical 3–5 g/day maintenance dose — given as a single acute dose, and the trial included only 15 people. Whether the same benefit shows up with routine low-dose or multi-day supplementation isn't something this study alone can answer.

Vegetarians take in less creatine from food
The body synthesizes some creatine on its own, but red meat and fish are also dietary sources. People who eat little or no meat or fish — vegetarians and vegans — take in less creatine from their diet. In a double-blind, placebo-controlled crossover trial of 45 vegetarians, 5 g/day for six weeks significantly improved performance on a working-memory task (backward digit span) and a reasoning task (Raven's Advanced Progressive Matrices) versus placebo. The trial did not measure brain creatine concentrations, so it does not establish that low brain creatine was the mechanism behind the improvement. What it supports is narrower: an improvement was observed in a group whose dietary creatine intake is low.

In healthy young adults, effects are small and inconsistent
A systematic review of 6 RCTs (281 participants) found improvements in short-term memory and reasoning tasks in older adults, but no clear change in cognitive task performance among healthy, well-rested young adults. The review also noted that doses, durations, and cognitive tests varied so much across the included studies that a quantitative meta-analysis (pooling effect sizes across studies) wasn't possible. Creatine isn't something that reliably sharpens a well-rested young brain.
Dosing, duration — and what the research doesn't show
Two quite different dosing schemes appear across these studies. The chronic trials used roughly 5 g/day for several weeks, which is close to the 3–5 g/day maintenance range used for training. The sleep-deprivation trial used a single 0.35 g/kg dose — about 25 g for a 70 kg person — which is an experimental bolus, not an extension of everyday maintenance dosing. Taking a maintenance dose does not mean you would see the sleep-deprivation effect. What none of this research shows is any effect on dementia prevention or Alzheimer's disease. Even for healthy brains, any benefit appears small and condition-dependent — that caveat should stay attached to every claim here.
